No Insurance: Student will not be able to take laptop home and if damage occurs, student is responsible for the costs associated with the damage.

Personal Insurance: Student will be able to take the laptop home after student provides proof of personal insurance to the technology coordinator.

School Insurance: Student will be able to take laptop home after the annual coverage fee has been paid and the laptop agreement form has been returned and signed. $30.00 per student of $60.00 for a family plan.

Permission Information

I knowingly and voluntarily release, acquit, discharge and waive, and further agree to indemnify, hold harmless, and reimburse the Ipswich Public School District 22-6, its officers, boards, agents and employees (Released Parties), and each and all of the Released Parties, from and against any claim which I, the Student or any other parent, relative or next of kin of ours, successors, or assigns or any other person, firm or corporation may now or hereafter have or claim to have known or unknown, seen or unforeseen, directly or indirectly, or within or without the control of the Released Parties including but not limited to those claims arising in whole or in part from the negligent acts or omissions of the Released Parties, and also from and against all claims which said Student or others hereafter may acquire, either before or after the Student has reached the age of majority, for or on account of any losses, damages, personal injuries, pain and suffering, death, property damage, contract claims, or negligence resulting from, or arising out of, directly or indirectly, during, or in connection with the Student’s driving from Ipswich Public School to other locations for purposes of participating in school day activities including but not limited to claims arising in whole or in part from the negligent acts or omissions of the Released Parties.

My child will be renting a horn from the Ipswich school. A $50.00 payment will be paid to the school by September 26th. I confirm that on receiving the instrument, the instrument was in sound repair and condition. I understand that this instrument is the property of Ipswich School District 22-6 and that it will be returned when the rental period is over in good condition. When instrument needs repair (cork or pad falls off, valve gets stuck…), the school is responsible and band director should be notified*

Yes - my child will rent an instrument and I understand full payment is due by September 26th and instrument is due back by May 1st.

Senior Parents Only -- I have read and fully understand the rules and regulations of the Senior Privileges Program as granted by the Board of Education of the Ipswich Public School. Students will receive Senior Privileges based on their ACT scores from their Junior Year, attendance, grades, and behaviors. Students can use Senior Privileges during Dual Credits hours and Study Halls. Seniors may drive to and from home at teh begginging of the day and end of the day as long as they dont have another class to attend during that time period (cant come to class, go home, and come back to class)..
