 If a medication is to be given because it was forgotten prior to the start of school, a school designee will call the parent to verify that the child has forgotten to take his/her medication that morning and for the parents’ permission to give the child one does of medication. In addition, if you the parent calls the school and tells us your child has forgotten to take his/her medication we will also administer the medication with parent consent.

 Prescribed medication MUST be provided in the current prescription container stating the name of the medication, student’s name, the name of the pharmacy, physician’s name and the dose to be given.

 Over-the-counter medications will be in the original store container and in an age appropriate form and dose.

 Consents for routine medications are good for one calendar school year unless otherwise notified by the parent. Cough medications will be good for a maximum of 2 weeks.

 The first does of any medication must be given by parent/guardian.

 The school will place each medication in a secured and locked cabinet at the school for safety and liability purposes.

 I absolve the school personnel of all responsibility for any unforeseen development/reaction due to the administration of the above name medication. It is the responsibility of the child to come to the office to take his/her medication. I understand that the school district and the individuals involved will not be held liable for any adverse effects of the medication.

 Students are prohibited from transferring, delivering or receiving any medication to or form another student. All violations will result in confiscation of the medication and disciplinary action will be taken in accordance with the district’s policies. Consents must be completed for each medication and a new consent must be signed as the over-the-counter/non-prescription
