Facility Use Agreement

All persons desiring to have access to the gym facilities (Ipswich auditorium and auxiliary gym) will need approval from school administration and are required to provide the following assurances and consents:

1. To provide at all time adequate and appropriate supervision by one or more parents/adults (at least 21 years of age) of any activity held in the gymnasium(s). Said supervisors will remain on-site until ALL participants have vacated and the building has been secured. A secured building means all doors checked/locked & lights off including bathrooms and locker rooms.

2. Neither the school district, school board, nor any officer or employee of the district is liable for any injury that occurs as a result of the use of the facility, regardless of the cause of the injury, including design, maintenance or condition of the facility and equipment used under this agreement.

3. The individual requesting usage is liable for damage to any person that may arise as a result of the use of these facilities, as provided in SDCL 13-24-20 and will also be liable for any damage incurred to the facility or equipment. Said individual will also be liable for the district’s legal expenses if any litigation results from the use of these facilities.

4. Notice is to be given no less than 24 hours in advance to personnel for intent of use of the facilities.

5. All gymnasiums must be cleared and facilities secured no later than 11:00 p.m.

6. All supervisors and participants must sign in prior to each gymnasium usage.

7. To inform school officials of concerns with any equipment or structural components.

8. To abide by all policies and rules of the school district.

9. Do NOT prop doors open with items found around the school. Mr. Knust or Mr. Osborne can open the facility with our phones.
